A form containing 224 questions to determine person's state of health in the context of nutritional deficiencies.
Once submitted, the questionnaire is evaluated within 24 - 36 hours and the results are emailed back to the sender with nutritional suggestions in the form of the specific whole-food nutritional formulations. At that time the person may order these formulations using the instructions in the email.

Shipping:

Same day shipping (UPS 2nd day) for all orders filled by 2 pm CT from existing customers. New customers same day shipping if orders filled by noon CT.
All orders up to $200 within the US incur $10 shipping charge. Orders above $200 -- no shipping charges. Orders to Canada incur a charge not to exceed $25 depending on the carrier regardless of the order amount.. Clients in other countries will be notified by email as to their shipping charges. Their permission is required to ship products. Customs fees and other taxes (if any) are the responsibility of the clients. Shipping charges quoted in US dollars.

Shelf Life of Supplements/Formulations:

Both nutritional and herbal formulations have at least 18-month shelf life from the date of delivery.
