IC	Initial consultation (office and telephone consultations*)	$60.00	40-minute consultation. Telephone connection fees: No connection fees to US and Canadian numbers and most European Union LANDLINE numbers. Calls to EU cellular numbers: $0.50/min. Skype computer-to-computer connections with or without video -- no connection fees.
GR	General rate (office and telephone consultations*)	$40.00	First 20 minutes, 10-minute increments thereafter ($20 per every 10 min). Telephone connection fees: No connection fees to US and Canadian numbers and most European Union LANDLINE numbers. Calls to EU cellular numbers: $0.50/min. Skype computer-to-computer connections with or without video -- no connection fees.

SUPP	Formulations, supplements	*******	Actual price plus shipping & handling (if apply). Patients will be informed before ordering.
MA	Missed appointment	$40.00	Clients with automatic billing arrangements: no charge if cancellation is of the emergency nature and scheduling of the new appointment occurs within 48 hours of the missed appointment; however, if the second appointment is missed, $80.00 will be charged. All others, charge of $40.00 immediately after appointment is missed.

*To set up telephone consultation send email or call the number at the bottom of this page. Have your preferred time of consultation and two alternatives. Have your credit card ready -- Visa, MasterCard. You will be charged $75.00 upon setting the appointment and balance upon completing the consultation. Telephone connection fee will be added (if applies).
